The original fix for this still had a race condition in certain
circumstances. The newest fix was done on the 13th of May, but
probably wasn't disted until today (QA, can you confirm this?). I'm
not sure if vsapd was restarted, so you should restart your vsapd just
to be sure:

  /usr/local/etc/rc.d/vsapd.sh restart

If this happens again, I'd very much like to know about it.
